Accommodated Exam Request Form

Please use this form to request a proctored exam through Academic Success and Accessibility. Complete all areas of the form. Your form must be submitted at least seven (7) days in advance of the exam date and with the consent of your instructor. You may submit up to three (3) exams on one form for the SAME course. Allow two (2) business days to process.

All proctored exams will be scheduled for the same day and beginning at the same time as the class exam unless requested and authorized by the instructor to schedule on a different day and time.

Please note our hours of operation are as follows:
Monday – Thursday: 8 a.m. – 9 p.m.
Friday: 8 a.m. – 5 p.m.

A different form must be filled out for each course.
