Practicum Documents & Forms

Clinical Mental Health Counseling Practicum Site Agreement

This document is an agreement between you, your practicum site, and the School of Education Office of Licensures and Credentialing. Once your practicum site is approved, you are required to sign the document and secure your site supervisor's signature before submitting the completed document.


Practicum Agreement Submission Form

This etrieve form is used to submit your signed Clinical Mental Health Counseling Practicum Site Agreement by the stated deadline for the term. 


Permission to Record Form

This Informed Consent form is used to obtain permisson to audio/video record your client(s). You should provide this form after disclosing to each client that you are a Counselor in Training.
