Welcome Packet Replacement Forms

We need to have have immunization, medical history, financial responsibility and health insurance verification forms on file for all new students.

Some forms can be filled out online and then emailed, mailed, or faxed.  Please follow these general instructions:

1. Be sure you have the Adobe Reader installed on your computer.
2. Click on the link to the form.
3. Fill out the form (except for the Immunization form).
4. Save the form to your computer (be sure to remember where it is saved!).
5. Attach and then email the saved form to the Health_center@redlands.edu.
6. The Financial Responsibility form must be printed, signed by a parent, then faxed or mailed to Student Accounts (information is on the form).
7. If you have difficulty attaching/emailing the form, you may print, then fax or mail the form to the location listed on the form.
